| Your Appointment: Arrival and Registration

The following tips and requirements will help make your visit a pleasant experience.
Arrival
- Please arrive 10 to 15 minutes prior to your scheduled appointment time to enable you to complete the registration process.
- When you enter our office, please sign in at the front desk. Our receptionist will confirm that you have an appointment scheduled and will ask you to go to one of our registration booths, or have a seat in our lobby until a booth is available. We offer complimentary coffee in the reception areas on all three floors. Please help yourself to a cup.
Registration
- Our registrar will ask you to confirm your personal information—such as date of birth, social security number, home address and telephone numbers—and make sure it is entered correctly in our system.
- Please have your insurance cards with you and readily available for copying. If your spouse is the primary card holder, please know his or her date of birth and place of employment.
- You also will be asked who your referring/family doctor is. Please have his or her address and phone number available, especially if your doctor is out of our area.
- You will be given the opportunity to sign an authorization allowing us to speak to family members or friends who assist in your health care. The form asks for their names and phone numbers and is entered into our system.
- You will then be directed to the floor or waiting area for your appointment. If at any time you require assistance getting to that area, we have wheelchairs available, and any one of our front office staff would be happy to escort you.
- The entire registration process should take only a few minutes if all your information is readily available.
Please note:
- On an annual basis you will be asked to sign our HIPAA privacy notice form that includes permission for us to treat you and bill your insurance company for services rendered.
- Co-pays are due at time of service and will be collected during the registration process. We accept payment in the form of cash, check, Visa, MasterCard and Discover.
